Clearance Information
The following documents are required for student-athletes to participate in athletics at a YUHSD school. Families are encouraged to upload completed forms to the student-athlete's Aktivate/Register My Athlete profile. Please follow the checklist below to ensure you are fully cleared to join athletic activities. If you have any questions, please contact the athletics office.

ATHLETIC CLEARANCE CHECKLIST
AKTIVATE/REGISTER MY ATHLETE
- Create account: https://registermyathlete.com/login
PHYSICAL DOCUMENTS
- Must use AIA Physical Form (must be correct school year)
- Must be completed annually
BRAINBOOK/OPIOID COURSE
- Students must complete their Brainbook and Opioid Course once during their 4 year high school career.
- Both courses are available at: https://academy.azpreps365.com
- Upload certificates to your Register My Athlete account under the Brainbook/Opioid section
ImPACT TESTING
- Required for participation in sports
- Completed with the school’s Athletic Trainer
- This is a computer-based cognitive baseline test that assesses memory, speed, reaction time, and problem-solving
- Testing is scheduled at the start of Fall, Winter, and Spring seasons
- Parents will be notified via ParentSquare when dates are confirmed
ATHLETIC PARTICIPATION FEE
- Pay after the athlete makes the team, but before the season begins
- Payments can be made at the bookstore (check, card, or cash) or online at: https://www.myschoolbucks.com
